filling in the form
What do these words/expressions mean? They could be found in the
forms.

1) issuing authority
2) second name initial
3) [home telephone preceded by] area/int'l dialing codes

And the second question: can we translate the word 'province'
as 'województwo'?

poszukaj pod Health Dept. w ksiazcze telefonicznej.
Najlepiej to skontaktuj sie z USCIS. Oni maja swoich lekarzy. Jak bedziesz
robila badania do GC to ten lekarz ci uzupelni brakujace szczepienia za jednym
zamachem i od razu wpisze do karty. Karta ta bedzie w zamknietej kopercie, do
ktorej ty nie mozesz zagladac....
USCIS maja liste lekarzy:
"The medical examination must be conducted by a civil surgeon who has been
designated by USCIS. For the most current list of civil surgeons in your area,
please check our Civil Surgeons Locator. To obtain by telephone the names and
phone numbers of the designated civil surgeons in your area, please call USCIS'
National Customer Service Center at 1-800-375-5283. You will be asked to
provide your zip code. Please have a pen or pencil ready to write down the list
of civil surgeons in your area."

Net calls get their own area code

Fixed phone firms must give access to emergency numbers
In the UK, the telephone area code for cyberspace will be 056.

Government regulator Ofcom has picked the prefix for customers who sign up
to make calls via the internet. Users can also opt for geographic numbers.

The decision on numbers comes as Ofcom reveals how it plans to regulate
services that use the net rather than the old fashioned telephone network.

Ofcom says it will use a light touch when regulating voice over net services
to help the new market flourish.

Call charges

"Broadband voice services are a new and emerging market," said Stephen Carter,
Ofcom Chief Executive. "Our first task as regulator is to keep out of the
way."

Services that route voice calls via the net, using a technology called voice
over IP (Voip), are becoming increasingly popular as more and more people
sign up for the broadband net services that they piggy back on.

As well as letting people talk via the net, voice over broadband services
also let companies offer all kinds of sophisticated message handling options
that are much trickier, and expensive, to do with the old-fashioned phone
network.

Getting an 056 phone number will also mean that you can use it where-ever
you are as long as you are connected to the internet.

Ofcom believes that the move to Voip will be broadly positive and could mean
call costs drop sharply because it costs a lot less to offer such services
than it does to set up an old-fashioned phone system.

Ofcom has also published a plain-English guide to Voip services to help
consumers find out what benefits they could get from these services and
what they need to do to sign up.

But as it was announcing the decision to adopt 056 for Voip users, Ofcom
also kicked off a consultation period designed to find out what it should
do to protect consumers using the new services.

There are a long list of obligations that anyone providing old-fashioned
phone services must comply with. Ofcom wants to find out if the same set
of obligations should apply to Voip suppliers.

For instance, phone suppliers must guarantee access to 999 emergency
numbers and Ofcom wants to see if the same responsibilities should
be placed on Voip firms.

The consultation period runs until 15 November.

Q: How does activation work?

A: Today's activation schemes work by creating a unique value or ID for each
user, based on the user's hardware configuration and the product serial number,
though this is implemented differently by different vendors. Microsoft, for
example, sends a single combined value, while Macrovision, maker of the popular
DRM product SafeCast, sends two separate values: the serial number and a number
representing the hardware. The user information is transmitted via the Internet
(or telephone) to a vendor's activation server, which receives the value,
records it in a database, and sends back a unique activation code that unlocks
the user's product.

Depending on the vendor's implementation, unique hard drive or Registry
identifiers are stored on users' machines. There is no way around this, since
the key to the process relies on checking user licenses locally each time the
application runs. Macrovision's SafeCast stores the values in several areas to
make cloning more difficult and to protect legitimate users from accidentally
wiping out their license files. One area is an unused portion of a PC's boot
track 0. While this sounds clandestine, an ID stored in that location is less
likely to be deleted inadvertently than a file or Registry entry would be.

Q: If Macrovision writes to track 0, won't that mess up security software,
partition tools, and other disk utilities?

A: Yes and no. A simple high-level format (Format C: /s) will not affect IDs
stored in track 0, but a low-level format (or repartitioning) will. Utilities
such as Norton Ghost can be used to back up a disk or partition to an image—and
restore the data with activation intact. But restoring an image that was made
before a product was activated will require reactivation. And while SafeCast
will not step on another SafeCast installation, products using other activation
schemes could be affected if they use the same location. In addition, security
products that watch for changes to hard drives could be affected as well
